Step 3. Submit an application for a voucher and documents to the authorized body

You can submit an application to the territorial body of the Federal Social Insurance Fund of the Russian Federation or to the social protection authority at your place of residence.

Note: To provide services for accepting applications and documents, an agreement must be concluded between the social protection body and the territorial body of the Fund on joint work to provide citizens with vouchers for sanatorium and resort treatment.

The application and documents can be submitted by the applicant (clauses 31, 47, 54 of the Administrative Regulations):

Step 4. Receive notifications about the registration of the application and the decision made

Within 10 days from the date of receipt of the application and certificate for obtaining a voucher, the territorial bodies of the Fund and the social protection authorities, as well as authorized bodies, inform the applicant of the registration date and registration number of the application (clause 3.8 of Procedure No. 328).

In the absence of grounds for refusal to satisfy the application, an official of the territorial body of the Fund prepares a draft decision on the provision of a sanatorium-resort voucher, which is agreed upon and signed by the head of the territorial body of the Fund (clause 62 of the Administrative Regulations).

A voucher for sanatorium-resort treatment is issued by the territorial office of the Foundation at the applicant’s place of residence.

Vouchers are issued in order of priority according to the date of application. A visit to the resort involves the child undergoing comprehensive treatment and restoring health. The child’s status is determined by the pediatrician, and only this specialist can write out referrals for the child to undergo sanatorium treatment. The document can also be issued by a specialist who is directly involved in the treatment of the child.
A medical observation plan in a medical institution is first drawn up, which necessarily indicates the need for treatment in a sanatorium. If there is such a recommendation from a doctor, the child will be able to undergo treatment free of charge in any of the sanatoriums of the Russian Federation. As a rule, vouchers are provided to children from 4 years of age, however, if a younger patient has chronic diseases and needs special treatment, a voucher can also be issued.
Algorithm for making a referral through a medical institution
If a child is on the list of persons who can apply for a voucher to the Mother and Child sanatorium, such a referral is issued free of charge from the clinic.
To apply for the benefit you will need to complete the following steps:
Contact your local pediatrician with an application for a voucher.
Pass a special medical commission, during which a decision will be made to allocate a referral to a sanatorium.
Take special tests: blood, urine, for enterobiasis, take a conclusion from a dermatologist about the absence of special skin diseases and get a certificate from a pediatrician stating that the baby has not had contact with sick people at home or at the place of education.
Issue a special certificate in form No.070/у-04, which is provided at the place of request and is stored in the sanatorium-resort institution for three years. The document is valid for six months from the date of signing.
Submit the collected papers to the Ministry of Health at the place of registration of the child, including the corresponding application.
Choose a sanatorium that suits your child according to all indicators from the proposed list of available diseases.
Get a sanatorium-resort card for your child, and then get a free trip. The documents are signed by the attending physician, the head of the clinic department, or the chairman of the medical commission.

If the child is not old enough to go to the sanatorium on his own, then the appropriate documents for the parent should be prepared. The greatest demand for vouchers is observed in the summer, so applications should be submitted in advance.

Registration of a voucher through social security
The “Mother and Child” project allows children, especially those in need of improved health, to go on vacation to a special institution. By analogy with the first option, a voucher is issued through social security.
The applicant must submit the following documents:
parent’s passport and child’s birth certificate;
documents confirming the presence of difficult circumstances in the family (certificates of parents’ salaries, statements of family composition);
medical certificate confirming completion of all examinations and tests;
compulsory medical insurance policy;
SNILS;
a certificate confirming the family’s status as a large family;
certificate of death of the breadwinner;
pensioner's certificate or certificate of assignment of a pension benefit for the loss of a breadwinner;
conclusion of the treating pediatrician;
a certificate in the prescribed form for obtaining a referral to a special institution.

The collected papers are submitted to the Ministry of Health 6 months before the date of the intended trip. Registration of a free voucher does not imply the receipt of benefits for travel expenses. As a rule, parents purchase tickets, but there are exceptions. For example, social security can reimburse expenses if a family’s income is below the current regional subsistence level.

a document containing information about the place of residence of a child, a person from among orphans and children left without parental care in the city of Moscow;
a document confirming the authority of the applicant as a parent (legal representative);
a document confirming the child’s preferential category;
a document confirming the preferential category of a person from among orphans and children left without parental care;
a document confirming the authority of the accompanying person for a joint holiday, including for an accompanying person by proxy;
a document confirming the authority to submit an application (if the application is submitted by an authorized representative);
insurance numbers of compulsory pension insurance (SNILS) of the persons specified in the application;
a document confirming the change of full name of the persons indicated in the application (only in case of change).

3. When to apply?

As a rule, the application campaign for free trips for the next year begins at the end of the current year. Thus, applications for free vacation trips for children in 2020 were accepted from November 2 to December 10, 2019.
At the second stage of the application campaign, which will take place from 10:00 a.m. on February 7 to 11:59 p.m. on February 21, 2020, you need to decide on the choice of a specific recreation and recreation organization. You can do this:

You have the right to refuse the provided voucher in two cases:
If there are at least 35 working days left before the arrival date. You can refuse by submitting a written application in person to the State Autonomous Institution “Mosgortur” (pre-registration is required).

illness, injury of a child;
illness, injury of an accompanying person (in the case of organizing a joint vacation);
the need for an accompanying person to care for a sick family member (in the case of organizing a joint vacation);
quarantine of a child or quarantine of a person living with the child, as well as, in the case of organizing a joint outing, quarantine of an accompanying person;
death of a close relative (parent, grandmother, grandfather, brother, sister, uncle, aunt);
children with disabilities and children with disabilities receiving sanatorium-resort treatment or rehabilitation at the same time as a free voucher for recreation and recovery.

The following have the right to payment of compensation for independently purchased travel vouchers:
