If it is impossible for legal representatives (father, mother, guardian, adoptive parents) to accompany their child to the hospital, to represent his interests when providing him with medical care (Federal Law No. 323 of November 21, 2011 “On the basics of protecting the health of citizens of the Russian Federation”), a power of attorney can be issued for a relative accompanying the child.
In accordance with current legislation, this power of attorney can be notarized or written in hand by the child’s legal representative.
When visiting the hospital, the authorized person must have with him:
original power of attorney
a copy of the power of attorney (provided once upon first visit
identification document (passport)
A copy of the power of attorney is attached to the child’s development history, and the original power of attorney is presented by the authorized person each time he visits any department of the hospital.

Also, for an outpatient card, a passport of the official representative and a birth certificate of the child are required.
Without a power of attorney, medical care will be provided only in urgent and emergency situations.
