A personal card is the main accounting document that contains the personal data of an employee. The form of personal card No. T-2 was approved by Decree of the State Statistics Committee of the Russian Federation dated January 5, 2004 No. 1. Personal cards for employees must be maintained by all companies, regardless of organizational and legal forms. Employee personal card is issued for each employee hired by the company under an employment contract. It is started upon hiring and maintained throughout the entire period of his employment in the company. With the dismissal of an employee personal card t 2 is closed, but continues to be stored in the company.

Is it possible not to issue personal cards, but instead to conduct personal affairs?

No you can not. The company is not obliged to draw up personal files for employees, while a personal card of form No. T-2 is a mandatory document. It is directly mentioned in the “Rules for maintaining and storing work books” (clause 12). It says that the personal card of the T-2 form repeats all the entries from the work book about hiring, transfers and dismissal. What documents are needed to fill out a personal card?

To fill out the T-2 form you need employee hiring order and personal documents of the employee, namely:

  • passport;
  • military ID (for those liable for military service);
  • document confirming graduation from an educational institution;
  • certificate of state pension insurance;
  • certificate of registration with the tax authorities;
  • other documents (for example, a medical record, driver’s license, documents confirming advanced training, state awards, disability, etc.). In addition, information will be needed that cannot be gleaned from documents. Contact telephone number, family composition, dates of birth of relatives, level of foreign language proficiency - the employee must provide all this information about himself. Therefore, it is necessary that he be present when issuing a personal card.
Do not trust filling out form No. T-2 to the employee himself, and fill it out only with your own hand. A personal card is a document of strict accountability and you bear responsibility for its correct execution.

The personal card consists of eleven sections:

In each of these sections there are columns where you need to record information about the employee. And in the first section (General Information) there are also windows for encoding information. Codes for filling out a personal card can be found in the information classifiers:

  • OKIN (All-Russian Classifier of Population Information OK 018-95), approved by Decree of the State Standard of the Russian Federation of July 31, 1995 No. 412;
  • OKSO (All-Russian Classifier of Specialties in Education OK 009-2003), approved by Decree of the State Standard of the Russian Federation of September 30, 2003 No. 276-st;
  • OKPDTR (All-Russian classifier of workers' professions, employee positions and tariff categories OK 016-94), approved by Decree of the State Standard of the Russian Federation of December 26, 1994 No. 367;
  • OKATO (All-Russian Classifier of Objects of Administrative-Territorial Division OK 019-95), approved by Decree of the State Standard of the Russian Federation of July 31, 1995 No. 413.
Is it necessary to encode information in a personal card?

Not necessary. Coding is intended for automated data processing and statistical accounting. It was introduced when universal computer programs for personnel officers did not yet exist. Using a computer that read codes from documents, it was possible to count the number of employees and divide them into categories (level of education, profession, etc.). Now such processing is practically not carried out anywhere. Even if personnel officers use special computer programs that automatically enter codes into a personal card, the codes themselves have lost their original meaning. But remember that columns cannot be deleted from unified forms. Therefore, if you are not encoding information, do not remove the code windows, but simply leave them blank.

Beginning of the form and section "General information"

First, the name of the company is indicated in accordance with the charter and other constituent documents. In the upper right corner you need to enter the company code according to OKPO (your chief accountant knows it). Write the date you filled out your personal card digitally, for example, 08/11/2006.

  • personnel number (it is assigned to the employee upon acceptance of the job and does not change during transfers and movements within the organization);
  • taxpayer identification number - TIN. If the employee has not been assigned one, leave the cell blank;
  • number of the state pension insurance certificate;
  • alphabet (first letter of the employee’s last name);
  • nature of work (permanent or temporary);
  • type of work (main or part-time);
  • gender (male, female).

Now we have to fill out the most voluminous section of the T-2 form - “General information.” First of all, in a special cell, indicate the number and date of the employment contract concluded with the employee.

POINT 1. LAST NAME, FIRST NAME, PATRONICAL NAME.” Carefully copy down the information from the employee’s passport. Full name and patronymic must be indicated. Try to leave space on the lines for future changes to your personal data (this is especially true if you are filling out a woman’s personal card).

POINT 2. “DATE OF BIRTH.” The month of birth is written in letters, and the date and year are written in numbers. On the right side in the code cell, write your date of birth digitally.

POINT 3. “PLACE OF BIRTH.” Recorded in accordance with the passport. If you are encoding data in the T-2 form, then to fill out point 3 you will need OKATO.

POINT 4. “CITIZENSHIP”. There are standard wordings for citizenship records. They are contained in OKIN. And even if you do not encode the information in your personal card, the entry in the text field of paragraph 4 must strictly correspond to this classifier.

What should I write in paragraph 4 of my personal card if the employee has dual citizenship?

In this case, indicate in brackets the second country of which the employee is a citizen. For example: “Citizen of the Russian Federation and a foreign state (Poland).” In the personal cards of foreign citizens it is also necessary to indicate the country: “foreign citizen (of the Republic of Belarus).”

POINT 5. “KNOWLEDGE OF A FOREIGN LANGUAGE.” When filling out this item, you must also adhere to the generally accepted formulations of OKIN, which are given in the table below. It is better to indicate the degree of knowledge of the language in the first person singular: “I speak fluently,” “I read and translate with a dictionary.” If you are putting down codes, keep in mind that there are codes not only for degrees of language proficiency, but also for the languages ​​themselves. For example, the English language code is 014, French - 213, Italian - 070, German - 135. Therefore, you need to enter both codes, separating them with several spaces.

POINT 6. “EDUCATION”. There are several levels of education (they are also called educational qualifications). Levels of education along with their designation codes are listed in OKIN. The table below shows educational levels with explanations and codes. If an employee has presented you with a certificate of postgraduate education, you will need to fill out the corresponding block in the “Education” section.” At the same time, remember that postgraduate studies, residency, and postgraduate studies correspond to code 01, and doctoral studies - code 02 (according to OKIN). Leave the OKSO code window blank, since OKSO does not have codes for postgraduate education specialties.

If a student who has not yet graduated from a university gets a job, how do you fill out the “Education” item in your personal card?

If an employee is studying in the fourth, fifth or sixth year of a university, a record of incomplete education is made on the basis of a certificate from the educational institution, student card or grade book. And if a student has just entered an educational institution and is studying in the initial courses, then the education that he has at the time of applying for a job must be recorded on his personal card. Let’s say a student graduated from high school before entering a university. This means he has a basic general education. The item “Education” is also filled out in the same way for students who have not yet graduated from an educational institution of secondary vocational education.

POINT 7. “PROFESSION”. This paragraph reflects information about what profession the employee has. Moreover, you need to enter not only your main profession, but also other professions (if any). The main profession is usually the one that is of greatest interest to the company, or the profession in which the employee has gained the most experience. To code your profession you will need OKPDTR.

POINT 8. “WORKING EXPERIENCE.” Experience - general, continuous and giving the right to benefits - is calculated on the basis of entries in the work book or other supporting documents (employment contracts, certificates from places of work, etc.). TOTAL WORK EXPERIENCE is calculated as follows: first you need to write out all dates of hiring and all dates of dismissal from the employee’s work book. Then add these dates separately. You will get two amounts - the first will consist of the dates of hiring, and the second - of the dates of dismissals. The next step is to subtract the first from the second amount. With this calculation, one day is lost in each period. This means that after the calculation, you need to add to the resulting number as many days as there were hiring and firing periods. If, as a result of calculating the length of service, a negative number of months or days is obtained, then from the number of years the breakdown is 12 months, and from the number of months - 30 days. The number obtained after all calculations is converted into years, months and days, taking into account that on average there are 30 days in a month, and 12 months in a year.

Engineer V.A. Danilov’s work book records three periods of hiring and dismissal:
- from May 27, 1996 to March 13, 2003, he worked as an engineer at FSUE lKontakt;
- from March 19, 2003 to January 31, 2004, he was a project manager at ZAO lAlbatros”;
- from February 1, 2004 to August 4, 2006, he held the position of head of a department at lRim LLC, and on August 11, 2006, he began working at lAlfa CJSC.”
Let's summarize the hiring dates:
05/27/1996 + 03/19/2003 + 02/01/2004 = 10/47/6003 Then we summarize the dates of dismissals from work:

We add 3 more days to the resulting number of days, since the employee had 3 hiring and dismissal periods. It turns out that the total work experience of V.A. Danilova is 10 years, 2 months and 4 days.

To calculate CONTINUOUS WORK EXPERIENCE, you need to check the duration of the breaks between the dates of dismissal and hiring. In this case, one must be guided by the “Rules for calculating the continuous work experience of workers and employees when assigning benefits for state social insurance,” approved by Resolution of the USSR Council of Ministers of April 13, 1973 No. 252. If an employee was fired for a culpable reason (absenteeism, showing up to work while intoxicated, etc.), the length of service is interrupted in any case.

Example

In the work book of engineer Danilov (see previous example), we see that he had three breaks between his dismissal and getting a new job: from March 13 to March 19, 2003, from January 31 to February 1, 2004, and from March 4 to August 11, 2006. In all three cases, the breaks did not exceed 21 calendar days. This means that Danilov’s continuous experience will be the same as his general one - 10 years, 2 months and 4 days. And if, say, an employee got a new job not on March 19, but on May 19, 2003, the length of service would be interrupted and begin to flow again. Then it would be equal to the last period of continuous work of the engineer - from May 19, 2003 to August 4, 2006 - and would be 3 years, 2 months and 17 days (calculation was made according to the scheme given in the previous example).

POINT 9. “MARRIAGE STATUS.” It is filled out on the basis of the passport, and if there are no corresponding marks there, then according to the employee. The OKIN contains standard formulations of a citizen’s marital status. They are shown in the table below along with the codes.

POINT 10. “FAMILY COMPOSITION.” As a general rule, the closest relatives indicated in this paragraph are considered to be parents, spouses, children, siblings, as well as persons in whose care the employee was or is.

ITEM 11. “PASSPORT.” This paragraph indicates the employee’s passport details.

POINT 12. ADDRESS OF RESIDENCE.” This paragraph contains very important information. You will certainly need it in order, for example, to find a missing employee, send him a notice or work book by mail, and also to urgently call him back from vacation. Ask the employee to provide as many contact numbers as possible - home, mobile, part-time work phone.

How to fill out clause 12 if the employee does not live where he is registered?

In this case, leave the line for the actual address empty - after all, the employee may well move without changing registration. Then you simply enter in this line the address at which he actually began to live.

How to correctly make corrections in a personal card? Is it possible to cover up inaccuracies with a stroke corrector?

No, information on a personal card cannot be covered with correction fluid, covered with paper, or erased with an eraser. If you made a mistake when filling out your personal card, it is better to redo it. And if an error is discovered some time after filling out and it is not practical to rewrite the card, correct the incorrect entry as follows. First, carefully cross out the incorrect entry with one line. Then, above or next to this entry, make the correct one and write the word “corrected.” Sign the correction and ask the employee to sign it. Don't forget to indicate the date of correction just below.

When filling out the “Place of Birth” and “Address of Place of Residence” items in the first section of your personal card, use generally accepted abbreviations for the names of address objects. They are shown in the table below.

Section "Information on military registration"

This is the only section, the completion of which is described in detail in the instructions for the use and completion of forms of primary accounting documentation for recording labor and its payment” (approved by Resolution of the State Statistics Committee of Russia dated January 5, 2004 No. 1).

All citizens entering work are divided into those liable for military service and those not liable for military service. In turn, those liable for military service are either citizens who have already served, who are in the reserves, or future conscripts. The section on military registration is filled out only for persons liable for military service.

The second section of the T-2 form is filled out differently for reserve officers and for those who have not reached the rank of officer. What military ranks are not officer ranks?

In the army, soldiers, sergeants and warrant officers are not officers, and in the navy - sailors, foremen and midshipmen (Article 46 of the Federal Law on Military Duty and Military Service" dated March 28, 1998 No. 53-FZ).

To fill out the second section of your personal card, you will need either a military ID (a temporary ID issued in place of a military ID) or an ID of a citizen subject to conscription.   • position (specialty, profession), rank, class (category) of qualifications;
  • salary or tariff rate, allowance.

In the “Bases” column, you must write the date and number of the hiring (transfer) order. And finally, you must ask the employee to sign in the column "Personal signature of the owner of the work book."

Section "ATTESTATION" must be filled out if the company conducts certification of employees (see chapter "Certification"). First, the date of certification is indicated, then the decision of the certification commission is briefly written down, and the date and number of the minutes of the commission meeting are also given. In the “BASE” column you should write the date and number of the order for certification. The certification commission not only makes a decision on the employee’s suitability or non-compliance for the position held, but also makes recommendations to the general director (for example, send the employee for training, increase his salary, transfer him to another position). These recommendations can also be reflected in this section.

The section “DEVELOPMENT OF QUALIFICATIONS” usually contains data on long-term training in an educational institution for further training (over 100 hours). After completing such training, the employee receives a certificate, certificate or diploma of the established form.

PROFESSIONAL RETRAINING is acquiring an additional profession. Moreover, we are talking about training on the basis of higher education and in the amount of at least 500 academic hours. The sixth section of the personal card is filled out on the basis of a diploma or certificate of professional retraining.

Does professional development include short-term training (one- or two-day seminars attended by the employee)?

Yes, it does ("Model Regulations on an Educational Institution of Additional Professional Education", approved by Decree of the Government of the Russian Federation of June 26, 1995 No. 610). However, it is not necessary to record absolutely all the seminars that he attended in the employee’s work book and personal card, otherwise there may not be enough space in the relevant sections.

How to fill out the seventh section of the personal card “Awards (incentives), honorary titles”?

This section takes into account both incentives from the company and government awards. In the appropriate columns you must indicate the name of the award, the number and date of the order or other award document (for example, a decree of the President of the Russian Federation on conferring a state honorary title). This section also duplicates information about awards from the employee’s work book. Clause 24 of the “Rules for maintaining and storing work books” provides a complete list of awards, information about which must be entered in the work book, and therefore in the employee’s personal card.

Sections “Vacation”, “Social benefits”, “Additional information”

In the "Vacation" section, all types of leaves are recorded (annual, educational, without pay, etc.). Remember that in the “Period of work” column, in any case, you must indicate the full working year for which the vacation is granted. The basis is either a vacation schedule or a vacation order.

If an employee takes part of the annual paid leave, then make a note about this in the “Type of leave” column. In addition, you need to mark on your personal card the moment when the vacation for a given working year is fully used. This will help you account for vacations more accurately.

If employees present to the HR department documents according to which they are legally entitled to any social benefits related to employment relationships, you have grounds to fill out the ninth section of your personal card “Social benefits.” This could be a certificate of disability, a certificate of the presence of a disabled child, a certificate of war veteran, a certificate stating that the employee is a donor, etc. We recommend that you indicate as a basis the normative act (law) that gives the employee the right to a benefit .

The section of the personal card “Additional information” is intended for information not included in other sections. Here you can mention a driver’s license, training that has not yet been completed, a foreign passport, etc.

While the employee works in the company, the last, eleventh, section of the personal card remains blank. And when he resigns, here you will need to enter the basis for dismissal with reference to the relevant article of the Labor Code of the Russian Federation, indicate the date of dismissal, as well as the number and date of the order to terminate the employment contract. Close the personal signature card of the personnel officer and employee.

The employee changed her last name due to marriage. To change the last name on a personal card, do I need to enter into an additional agreement with her to the employment contract?

No, don't. Since the employee’s surname is not an essential condition of the employment contract, but refers to information about the employee, changing it does not entail the obligation of the parties to enter into an additional agreement. In this case, in order to make changes to personnel documents (personal card, time sheet, etc.) and accounting documents, an order is issued to make changes to these documents in connection with the change of the employee’s last name. The basis for issuing the order is the marriage certificate issued by the registry office.

When issuing an order to change an employee’s last name in a personal card and other accounting documents, do not use the wording: “on such and such a date, Ivanova shall be considered Petrova,” since it does not reflect the essence of the changes.

You also cannot write: “Change the surname of the senior inspector of the personnel department Ivanova to the surname of Petrov,” since the surname of a citizen is changed by the registry office department, and not the personnel service. The correct wording of the order to change the surname (first name, patronymic) sounds like this: “Change the surname of the senior inspector of the personnel department Maria Ivanovna Ivanova in the accounting documents to the surname of Petrov. Reason: personal statement of the senior inspector of the HR department Ivanova M.I. and a copy of the marriage certificate.”

The employee's personal card has undergone natural aging. Is it possible to get a new personal card?

You can re-register it. A personal card form T-2 is issued when an employee is hired. And in accordance with paragraph 12 of the “Rules for maintaining and storing work books,” you are obliged to familiarize the employee with each entry made in the work book against a signature in his personal card. When personal cards become dilapidated and unusable, the decision to reissue them is made by the head of the company. Such a decision must be formalized by order of the manager and familiarized with it to all employees against signature.

How to store employee personal cards in the HR department?

Since personal cards contain personal data of employees, you need to store them in a safe or cabinet that can be locked with a key. Free access of personnel to these documents must be excluded.

Personal cards of working employees are systematized using separators by structural divisions, and internally - alphabetically. The personal cards of those dismissed are confiscated and stored separately. They are arranged by year of dismissal, and inside - also alphabetically. The storage period for personal cards of dismissed employees is 75 years.

Abbreviations are allowed when indicating place of residence or information about education, but in a generally accepted form that does not distort the meaning of the information.

The completed T2 form must contain codes corresponding to the all-Russian classifiers: OKIN, OKATO, OKSO, OKPDTR. The number designation must be entered in a special table on the right side of the line. All code designations are freely available in printed reference books, as well as on the All-Russian Classifiers website.

If you fill out the classification code incorrectly, the T2 form is considered invalid, in which case you will need to create a new personal card. Strikethroughs and corrections in the coding table are not allowed.

Changes in the employee’s work activity or personal data must be reflected in a timely manner in T2. All changes made to the personal card are certified by the signature of a specialist from the HR department.

Information is entered into T2 on the basis of supporting documents provided by the employee.

The words and phrases “no”, “wasn’t”, “not available”, as well as dashes are not included in the T2 form - such lines are left blank.

Grounds for entering information about employees

The T2 personal card must be filled out based on the following documents:

Employment contract with the employee;

Orders on the organization: on hiring, transfer, granting leave, dismissal;

Passport (other document replacing it) of the employee;

Documents confirming the education received, courses completed, etc.;

TIN certificate;

Military ID.

Other information (for example, the degree of proficiency in foreign languages) is indicated only from the employee’s words. If necessary, the employer may request other supporting documents, in accordance with the specific professional orientation of the organization, as well as the established internal regulations.

How to fill out the title?

The first page of the form contains information about the employee and his level of professional qualifications. Filling out the T2 card (a sample of entering information is presented below) begins by indicating the name of the employer's organization in the top line of the form. The name is indicated in full, in accordance with the statutory documents of the organization. The coding table contains the digital designations OKPO and OKUD. Next, the following data is entered into the table below:

Date of filling T2 (Arabic numerals);

Personnel number assigned to the employee upon employment;

TIN (based on the Federal Tax Service certificate);

SNILS of the employee;

In the “alphabet” column the first letter of the employee’s last name is entered;

Nature of work, in accordance with the terms of the employment contract: temporary, permanent;

Type of work: can be either “main”, or for external or internal part-time workers - “part-time”;

Gender is indicated by the letters "F" or "M".

After completing the title of the document, you can move on to other sections, including the initial filling out of the T2 card. Example of entering information into the header:

General information about the employee

Completing this section begins by indicating in the “Employment Agreement” table the number and date of the employment agreement concluded with the employee. Next, filling out the T2 card, a sample of which is presented below, is carried out in the following order:

1. Line “1” indicates the full last name, as well as the first and patronymic of the employee.

2. In line “2” the date of birth is indicated in text form. Example: "April 20, 1981." In the table, the same date is encoded in Arabic numerals: “04/20/81”.

3. Line “3” must contain information about the place of birth. It is important to be guided by the following requirements:

When indicating a populated area with the words “city”, “village”, “region”, “village”, “district”, “region”, generally accepted abbreviations are allowed: “region”, “kr.” and so on. Example: “hor. Samara", "village. Taiga". However, it is recommended to indicate the designations “stanitsa”, “kishlak”, “village”, “aul” in full.

The total number of characters in line “3” should not exceed 100 characters.

The coding table is filled out in accordance with the All-Russian Classifier of Territorial Entities (since 2014 - new name code OKTMO).

4. Line “4” is intended for entering information about the employee’s citizenship based on the passport. All subsequent information in this section is entered in strict accordance with the OKIN code. Please enter information as follows:

Citizen of the Russian Federation (the designation “1” is entered in the encoding table);

Code “2” is indicated if the employee has dual citizenship, one of which is Russian citizenship; the entry about this looks like this: “Citizen of the Russian Federation and a foreign state”;

For employees who are citizens of another state, the code “3” is indicated in the coding table; the entry “Foreign citizen” is entered in line “4”;

If the employee is stateless, “Stateless person” is entered in the line, “4” is entered in the coding table;

5. Line “5” indicates the employee’s knowledge of a foreign language and level of language proficiency. The OKIN coding assumes three categories characterizing the level of proficiency in writing and speaking in a foreign language:

Code “1” - translates and reads with a dictionary;

Code “2” - reads and can be explained;

Code "3" - fluent.

In addition, OKIN contains specific codes for foreign languages, which must also be indicated in the coding table. Thus, the information in line “5” is entered in the following order: name of the foreign language - level of proficiency - code designation of the language and degree of proficiency (in the table). Filling out the T2 card, sample: “German - Fluent” OKIN code: 171/ 3.

6. The sixth line of section “I” is filled out solely on the basis of educational documents provided by the employee. An example of a text designation and the corresponding OKIN codes are presented in this table:

Further, in the table below, you should indicate: the name, series and number of the education document provided by the employee, the year of completion of training, as well as the qualifications obtained and the name of the profession in exact accordance with the document. The profession code according to OKSO is entered into the table. If an employee has provided two documents on education, for example, secondary vocational and higher education or first and second higher education, information about all available documents is entered in T2. Similarly, sections IV - VI contain information about retraining, certification, and advanced training received by the employee after employment.

Information about incomplete education should also be entered in T2, indicating the type of education available. For example, for an employee undergoing training at the time of employment, information about the education that he had before entering the current educational institution should be entered; based on a certificate from the educational institution, indicate that the employee is currently studying, the name of the educational institution and the course of study. Information is entered in a similar way if the employee underwent training, but did not complete his studies, but was expelled (or was expelled):

If less than three courses have been completed, only the education that the employee had before admission should be indicated;

Information about completed training, subject to completion of three or more courses of an educational institution, is entered into T2 on the basis of a diploma (certificate) of incomplete higher education; in this case, the designation “Incomplete higher education” and the course from which the employee was expelled are indicated;

7. Lines “7” indicate the full name of the main and, if any, additional position (profession) of the employee, which he occupies in the organization in accordance with the hiring order and staffing table. Abbreviations or distortions of the name are not allowed. The section coding table indicates the job code according to OKPDTR.

Work experience

It is entered in accordance with the employee’s previous work activity, the basis is the work book. If an employee is employed for the first time, zeros are entered in the first line “general”.

Information about the employee’s relatives: who should be indicated?

Completing this section begins with indicating the employee’s marital status at the time of employment. Words and phraseological units “married”, “in a civil marriage”, “cohabiting”, etc. should be avoided: the line is filled with the wording determined by the all-Russian classifier indicating the OKIN code.
If the marital status changes, amendments are made on the basis of a registry office certificate, indicating the details of the document.

Information about relatives directly living with the employee, indicating the degree of their relationship, should also be included in such a document as the employee’s personal card. A sample of filling out T2 looks like this:

Military registration

Information on suitability for military service is filled out on the basis of the provided military ID, indicating the rank and reserve category noted in the document. Information for items 3-4 is contained on page 3 of the military ID. For an employee who has not yet served, but has no restrictions on service, the lines are left blank, with the mark “subject to conscription” being entered in line 2.

The degree of suitability for service is indicated on the basis of the registration certificate by the letter designation:

Fit with restrictions - B.

Limited fit - V.

Temporarily unfit - G.

Line 8 of the section is intended for the marks “Removed from military registration” indicating the reason, for example, due to age or health status.

Page 3, section III

This section exclusively contains information about the transfer of an employee to another structural unit within the same organization (including transfer to another location) or about a change in job functions. All information is entered on the basis of orders for the organization, indicating the details of the order (date, number).

Important to remember! Information about changes in rate or salary made without changing the employee’s position should not be indicated in this section.

The employee must be familiarized with all entries made in this section against signature.

Employee holidays and rest days

This section is filled out as the employee is granted leave in accordance with labor legislation and internal regulations. In the section table you must enter the details of the vacation order, type of vacation, and number of calendar days. For annual leave, you should also indicate the period for which vacation days are given in columns No. 2, 3 of Table VIII (employee’s personal card). The sample filling looks like this:

Section X: What information can I provide?

“Additional information” is a section provided for information that may be related to work activity, but is not indicated in other sections of the T2 form. Such information may include, for example, whether the employee has driving experience and a driver’s license of a certain category, health restrictions (in accordance with a medical report), etc.

How to correctly enter information about dismissal

The last section is completed on the employee’s last day of work. You should indicate the details of the order, the reason for the employee’s dismissal and confirm the entry made with the signature of a personnel employee. The employee must also be familiarized with the records of Section XI by signature.

An employee’s personal card is a document that contains basic information about the employee, his education, work history, awards, etc. It is filled out when hiring, but since it is a fairly voluminous document, HR specialists often have questions about its preparation. Let's look at what the T-2 card form looks like and where to get the form of this document, and also show an example of filling out a T-2 card.

What is this form and when is it needed?

Before talking about a sample of filling out the T-2 form, we note that its form is approved by Resolution of the State Statistics Committee of Russia dated January 5, 2004 No. 1. The document is kept for each individual employee. This is convenient because it allows the employer to have a complete dossier on the employee, so to speak. The fact is that the following information is recorded on the card:

  • about the date and place of birth;
  • citizenship;
  • education, including additional education, and professional training;
  • knowledge of foreign languages;
  • work experience;
  • family composition;
  • registration and residence address;
  • attitude towards military service;
  • certification results;
  • awards;
  • vacations;
  • benefits;
  • other information.

All this data is accumulated by the employer to fulfill the obligations provided for by law. For example, information about family composition will allow the employee’s children to be included in the list of recipients of New Year’s gifts or vouchers to summer camps. Information about additional education or knowledge of foreign languages ​​will make it possible to offer the employee another position or send him on a business trip abroad. Data on awards and length of service can be considered as a basis for sending a colleague’s documents for awards at a higher level. So an employee’s personal card is a very convenient and useful source of information for employers.

Who is required to maintain personal employee records?

Some employers believe that if the primary accounting forms given in the Resolution of the State Statistics Committee of Russia are not mandatory, then they may not exist in the organization at all. However, not all so simple. The Labor Code, indeed, does not directly oblige managers to maintain and fill out these forms. However, paragraph 12 of Government Decree No. 225 dated April 16, 2003 “On Work Books” states that the employer is obliged to familiarize the employee with all changes made to the work book. In this case, the employee must confirm his consent by signing a personal card, in which all entries made in the work book are duplicated.

In addition, you need to know that the T-2 form is one of the military registration documents, according to paragraph 27 of the Decree of the Government of the Russian Federation of November 27, 2006 No. 719. Thus, maintaining this document is mandatory for all organizations and entrepreneurs. General rules for filling out form T-2

A small note before telling and showing how to fill out the T-2 card and a sample of filling out each item. There is a standardized form that is recommended to be used. The employer has the opportunity to change it, or rather, supplement it with information that, in his opinion, is missing, but nothing can be removed from it. At the same time, it is important to understand that the form approved at the state level was developed taking into account the protection of the citizen’s personal data. And the employer, if he wants to make additions to the T-2 form, must also take into account the specifics of the legislation in this area. An example of filling out a T-2 employee’s personal card, which will be presented in this article, is given on the basis of a unified form.

The next point you need to know is that the personal card is kept by a person authorized to maintain personnel records in the organization. This is done upon hiring within a week. The document is issued exclusively in paper form. If you are comfortable duplicating information from it in electronic format, you can do so. But while digital signatures are not common in Russia within the framework of labor relations, and employees sign with their own hands, T-2 cards must be kept on paper and all data must be entered into them manually. In this regard, we note that there are no requirements for the color of the ink used.

As for errors and corrections, unfortunately, they cannot be avoided. If an inaccuracy was made when filling out, the incorrect entry must be crossed out with one line (the entry must remain readable), write the correct wording next to it, then add “Believe the corrected” and sign.

Sample of filling out the T-2 card

Let's consider a specific example of filling out an employee's personal card (form T-2 2019). Let’s say that economist Ivan Ivanovich Ivanov gets a job at Svetly Put LLC in February 2019. When hiring a new employee, the following documents will be required (based on them, data will be recorded in the form):

  • identification;
  • work book (if available);
  • SNILS;
  • military registration documents;
  • documents on received education and qualifications;
  • other papers, if they are required for admission to perform a job function.

The personnel officer also needs a hiring order.

When all the papers are collected, you can begin to fill out the form. A sample of filling out a T-2 employee personal card will be presented step by step, but if you wish, you can download the fully completed document at the end of the article.

We start filling out the form from the header. Here you should indicate:

  • full and abbreviated name of the employing organization;
  • date of drawing up the card;
  • personnel number, INN (if available) and SNILS of the new employee, his gender (abbreviated as “M” or “F”);
  • the nature of the work - permanent or temporary;
  • type of work - main or part-time. In this cell, words must be written in full; abbreviations are not allowed;
  • the “Alphabet” field is filled in with the letter with which the employee’s last name begins (in our example it is “I”).

Section I is called "General Information". Here are the following:

  • number of the employment contract and the date of its conclusion;
  • FULL NAME. employee, date and place of birth, citizenship;
  • degree of proficiency in a foreign language (one or more);
  • information about education and profession.

Already from the second line, HR officers may have difficulties because they do not understand what codes need to be entered and where to get them from. OKATO code is the number of a settlement (its district) in the All-Russian Classifier of Objects of Administrative-Territorial Division. Changes are often made to it, so when filling it out, it is advisable to find out accurate information from trusted sources.

As for the abbreviation OKIN, it stands for All-Russian Classifier of Population Information. From this list of codes, which are divided into 293 categories, information is taken to fill out. For the convenience of readers, we provide a summary table of codes that will be needed when creating lines 4, 5 and 6.

Information about education must also be supplemented with details from the diploma: who issued it and when, in what specialty. At least one line must be filled in. If in the future the employee brings additional education diplomas, this information will need to be added.

When filling out the tabular part, you will also need a code from OKSO - a classifier of specialties by education. In this part, you can write words abbreviated, but in such a way that the entry remains understandable. For example, you can write “to them.” instead of "name".

In the next line - the seventh - you must indicate your profession. To do this, you will have to use OKPDTR - a classifier of worker professions, employee positions and tariff categories. The profession of economist has code 27728.

In the “Work Experience” field, you need to fill out all the lines, if possible. If not, then fill out only the first line - about the total length of service at the time of concluding the employment contract from the work book.

Let us remind you that the total length of service is the period of activity that gives the right to an old-age pension. Continuous takes into account only the period that is used when assigning benefits, and the length of service that gives the right to a long service bonus is work in certain government organizations in certain government positions (a detailed list is presented in Section II of the Regulations approved)
